It is rather hard to tell, but it looks like one tongue is hanging down and the other one is slightly sticking out and on the top side of the mouth. The bottom head was shot using a flash causing the eye retina to reflect while the other (buck) doesn't have this. Then there is the shadow between the two heads that doesn't appear anywhere else. I am voting with the majority that this is a fake.
